Overview
Tesamorelin is a modified GHRH analog with a defined branded indication for excess abdominal fat in adults with HIV-associated lipodystrophy. Imaging-based regional-fat outcomes are central to its evidence1,5.
The approved label expressly excludes general weight-loss management because the branded treatment is weight neutral. The compounded catalog preparation is not EGRIFTA, and its clinical equivalence is not established by sharing the molecule’s name5.
Development & clinical specificity
The analog was developed to stimulate endogenous GH through the GHRH receptor. Clinical development focused on a population in which excess visceral fat and metabolic changes posed a particular problem1,2,5.
The resulting trials measured visceral adipose tissue with imaging rather than relying only on body weight. That choice allows a precise finding, but also restricts how broadly the outcome can be generalized1,2.
Mechanism of action
Receptor stimulation raises endogenous GH and downstream IGF-I. These are the same growth-factor systems that explain both the metabolic rationale and important monitoring concerns1,5.
Reduction in visceral fat is not equivalent to removal of all belly fat. The pooled trials found no significant change in abdominal subcutaneous fat, and long-term cardiovascular benefit was not established1,5.
- 1Mimics the body’s GHRHTesamorelin is a stabilized analog of growth-hormone-releasing hormone that activates pituitary GHRH receptors5.
- 2Restores natural GH releaseRather than supplying GH directly, it prompts the pituitary to release the body’s own growth hormone, raising IGF-I1,5.
- 3Targets visceral fatRaised GH signalling preferentially reduced visceral adipose tissue, the deep abdominal fat linked to metabolic risk, measured by CT in phase 3 trials1,2.
- 4Clears liver fatIn adults with HIV and fatty liver disease, 12 months of treatment produced a 37% relative reduction in liver fat3.
- GHRH / GRF receptor
- Pituitary somatotrophs
- Endogenous GH
- IGF-I
- Visceral adipose tissue
Pivotal & related trials
The pooled phase 3 program included 806 participants and assessed visceral adipose tissue at 26 weeks. The placebo-adjusted treatment effect was -15.4%; this is a regional-fat effect, not a percentage loss of total body weight1.
A separate HIV-associated fatty-liver trial reported a -37% relative liver-fat treatment effect, corresponding to a -4.1 percentage-point absolute effect. Liver-fat measures and approved lipodystrophy use are distinct endpoints and indications3.
The longer safety-extension trial found that improvements were rapidly lost after switching from tesamorelin to placebo. A finite course therefore cannot be promised to permanently reset visceral-fat biology2.
Visceral fat reduced1
Pooled phase 3 trials in 806 adults with HIV-associated abdominal fat found a 15.4% placebo-adjusted visceral-fat reduction at 26 weeks.
Results that hold2
Patients who continued treatment for 12 months maintained an approximately 18% reduction in visceral fat.
Normalized liver fat3
After 12 months, 35% of participants on tesamorelin reached liver fat below 5%, versus 4% on placebo.
FDA-approved molecule5
Tesamorelin is approved as Egrifta to reduce excess abdominal fat in adults with HIV-associated lipodystrophy.
GH-axis monitoring
The branded label addresses active malignancy, elevated IGF-I, fluid retention, glucose intolerance or diabetes, hypersensitivity and pituitary-axis disruption. A favorable average trial glucose result does not eliminate an individual’s glycemic risk5.
Pregnancy is a contraindication in the branded label. Long-term cardiovascular safety is unestablished, and formulation-specific storage and reconstitution differ even between branded variants5.
- The compounded preparation is not FDA-approved EGRIFTA.
- Active malignancy, pregnancy and pituitary-axis disruption are branded-label contraindications [5].
- IGF-I, glucose changes and fluid retention require monitoring [5].
- Hypersensitivity and injection-site reactions can occur [5].
- Regional visceral-fat data do not establish general weight-loss or cardiovascular benefit.
Supply, storage & administration
EGRIFTA WR is supplied as lyophilized powder for subcutaneous use, with its own compatible diluent and handling requirements5. This compounded vial is not that brand; verify its final form and instructions instead of assuming another tesamorelin formulation’s reconstitution method.
The catalog specifies vial content, not a verified final formulation or storage specification. A lyophilized preparation requires the pharmacy-specified compatible diluent; a ready-to-use liquid does not. Follow the dispensing label for refrigeration, light protection and the beyond-use date, and use aseptic technique with a new sterile needle and syringe6. Dosing is set by your provider.
Questions
Tesamorelin or TRZ-2?
Tesamorelin’s branded indication concerns HIV-associated abdominal lipodystrophy and visceral fat. Tirzepatide’s branded programs include body-weight outcomes in obesity.
Is the fat figure total weight loss?
No. It is an imaging-derived visceral-fat treatment effect relative to placebo.
Will it remove subcutaneous belly fat?
The pivotal pooled trials did not show a significant effect on abdominal subcutaneous fat.
Is the effect permanent?
Withdrawal findings showed return of visceral fat after stopping; durability requires assessment.
Can EGRIFTA instructions be used for this vial?
No. Branded formulations themselves have different reconstitution and storage requirements, and this compounded product needs its own instructions.
